Checkout Fields Manager is an extension for Easy Digital Downloads that lets store owners collect the information their business needs through custom checkout fields. Using the drag-and-drop interface, you can add text fields, drop-downs, checkboxes, file uploads, and other field types. It supports 22 field types, including select, multiselect, radio, country, date, URL, repeat fields with multi column support, section breaks, terms and conditions, hidden fields, and an action hook.

Conditional logic works on almost all form fields, so the checkout form can change based on the cart total, the items in the cart, or the user's behavior, and information is only requested when it is needed. Typical uses include collecting company names, bios, and business contacts, gathering design proposals and mock-ups when selling custom services such as logos or t-shirts, and adding a notes or special requests field to orders.
